fold increased risk of developing enterop− athy−associated T−cell lymphoma com− pared with the general population [1]. Early diagnosis is therefore required to improve their prospects. Indigo carmine chromoendoscopy highlights mucosal ir− regularities and improves the detection of malignant lesions [2]. Furthermore, it can delineate villous atrophy in celiac dis− ease [3]. This method is commonly used in upper and lower gastrointestinal en− doscopy but this is not yet the case in small−intestinal endoscopy. In this report we describe the chromoendoscopic ap− pearances of jejunal enteropathy−asso− ciated T−cell lymphoma, before and after indigo carmine spraying, in a patient who was undergoing a double−balloon endoscopy procedure. A 68−year−old Caucasian woman with ce− liac disease who had been on a gluten− free diet for 3 years was referred for dou− ble−balloon endoscopy because an ab− dominal computed tomography scan had shown thickened small−intestinal wall. Endoscopy was performed with a dou− ble−balloon endoscopy system (FMH Medical Inc., Veenendaal, The Nether− lands). Using a catheter, 10 mL of a 0.1% solution of indigo carmine was sprayed onto the jejunal mucosa on withdrawal of the endoscope. Scalloping or loss of folds, a mosaic ap− pearance of the jejunal mucosa, and round or circumferential ulcers (10± 90 mm along their longitudinal axis) were detected with standard video en− doscopy, but the villi were easier to re− cognize after indigo carmine staining of scalloped jejunal folds (l" Figure 1) and ulcers were also highlighted (l" Figure 2, l" 3). Histological examination revealed the presence of enteropathy−associated T−cell lymphoma in biopsies taken from the ulcers and partial to subtotal villous atrophy in the nonulcerated mucosa. In comparison with standard endoscopy, indigo carmine chromoendoscopy high− lighted areas of villous atrophy [4] and ul− Indigo carmine chromoendoscopic appearances of enteropathy−associated T−cell lymphoma during double−balloon endoscopy in a patient with celiac disease

Refractory Celiac Disease

Refractory celiac disease (RCD) is when malabsorption symptoms and villous atrophy persist despite strict adherence to a gluten free diet (GFD) for more than 12 months and other causes of villous atrophy have been ruled out.  RCD is considered a rare disease and almost exclusively occurs in adults. Persistent diarrhea, abdominal pain, weight loss are the most common symptoms in RCD. Also, anemi...

Endoscopic findings of enteropathy-type T-cell lymphoma.

Enteropathy-type T-cell lymphoma (ETL) is a rare primary intestinal disorder, particularly in Japan, and there have been few reports on the endoscopic findings of the disease. Here we report detailed endoscopic findings of ETL based on double-balloon enteroscopy and capsule endoscopy. Double-balloon enteroscopy and capsule endoscopy may be useful tools for diagnosing and monitoring the effects ...

Intraluminal injection of indigo carmine facilitates identification of the afferent limb during double-balloon ERCP.

Double-balloon endoscopy facilitates endoscopic retrograde cholangiopancreatography (ERCP) in patients with a Roux-en-Y anastomosis [1]. However, it is difficult to identify the afferent limb during double-balloon ERCP [2]. According to retrospective data from February 2007 to January 2009, the rate of accurate identification was 50% (14/28). We report a useful technique to identify the afferen...

Multiple lesions of gastrointestinal tract invasion by monomorphic epitheliotropic intestinal T-cell lymphoma, accompanied by duodenal and intestinal enteropathy-like lesions and microscopic lymphocytic proctocolitis: a case series

BACKGROUND In East Asia, monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL), previously known as type II enteropathy-associated T-cell lymphoma (EATL), occurs more frequently than type I EATL, and coeliac disease is rare. CASE PRESENTATION Here we present four cases of MEITL in Japanese patients, including the endoscopic and pathological findings of their duodenal and colorectal l...

Type II Enteropathy-Associated T-cell Lymphoma: A Rare Report from Iran

Type II enteropathy-associated T-cell lymphoma (EATL) is a rare disease that is reported from Asia, especially from Taiwan. In this paper, we present a patient from Iran (Arab) with Type II EATL whose disease was not associated with celiac disease or Epstein-Barr virus (EBV) infection. The patient died due to intestinal perforation during the first chemotherapy. Type II EATL is a gastrointestin...
